General Practitioners - Referrals
Information to include in vascular referrals.
Aneurysms
- State the location, e.g. aorta, iliac, popliteal artery.
- The size of aneurysm diameter, either anterior-posterior or transverse.
- Include any copies of ultrasound or CT that may already be performed.
Carotids
- State whether the patient is symptomatic or asymptomatic.
- If symptomatic, the date and description of symptoms.
- Include any copies of ultrasound or CT that may already be performed.
Peripheral vascular disease
- State whether patient has claudication or critical limb ischaemia (ulcers, rest pain).
- If claudication then the distance they can walk and level of impairment it causes.
- If ulcers or tissue loss, state location and size of involvement.
Varicose veins
- Specify location as this will enable pre consultation scans.
- If they had any complication from varicose veins such as ulcers, recurrent cellulitis, phlebitis and bleeding.
- Previous surgery, and any history of DVT or thrombophilla.
In all patients
- Medical history, especially cardiac, respiratory, renal and smoking
- Their functional level, e.g. independent, rest home with one person assist.
- BP, pulses and medications. If on Warfarin the indication.
- Lab results of creatinine, HbA1c and lipids if available
